Please assist us by providing the following information at the time of your initial consultation:

•  A referral slip from your doctor and any other important communications.

•  A list of prescription, over-the-counter and herbal medications you are presently taking.

•  If you have medical or dental insurance, please bring your insurance card.

•  Please take advantage of our online forms for patient registration. This will save you time on the day of your appointment.

IMPORTANT: All patients under the age of 18 must be accompanied by a parent or guardian at the initial consultation visit.

A pre-operative consultation is required for patients undergoing intravenous anesthesia for surgery. Please have nothing to eat or drink including water for at least 6 hours prior to your surgery. You will also need an adult to drive you home.

Please alert our staff if you have a medical condition or if you are currently taking any medication.

If your dentist or physician has taken x-rays, you may request that they forward them to our office. If additional imaging is necessary, it can be performed at our office.
